Your 90-Day Maintenance Review

Use it to make the next ninety days visible. A worksheet cannot prevent regain. It can keep the next conversation specific.

Your definition of maintenance. A weight range, a function goal, laboratory targets, how clothes fit, or another measure you and a clinician can both recognize. Write it in one sentence.

The trend. How you will measure, how often, and what "rising" will mean. Prefer repeated measurements over a single reading. If weighing is harmful, write the non-scale signals you will use instead.

Medication and access. What is continuing, changing, or at risk — coverage, supply, cost, tolerability, pregnancy planning — and who you will contact if that changes.

Hunger and satiety. What louder hunger or quieter fullness will look like for you, without treating either as a moral event.

Meals, strength, movement, sleep, and stress. What you can actually keep for ninety days, not an ideal week copied from someone else.

Follow-up and questions. The next clinician touchpoint, and three questions you will bring.

30 / 60 / 90. At day 30, check the trend and whether the plan is still possible. At day 60, check what broke and what held. At day 90, decide with a clinician whether the definition of maintenance still fits.
