Chapter 8

Want Children? Read This Before Starting TRT

This may be the most important chapter in this book for younger men.

TRT Can Lower Your Sperm Count

Your brain sends signals to your testicles.

Those signals tell your testicles to make:

  • Testosterone
  • Sperm

When you take testosterone from outside your body, your brain may sense that there is already enough testosterone.

It then reduces those signals.

Your testicles may make much less sperm.

In some men, sperm production can stop almost completely.

That means TRT can make it much harder to father a child.

How TRT can lower sperm count

Brain
LH and FSH signals can fall.
Testicles
Internal production can slow.
Sperm
The sperm count can become very low or reach zero.

Testosterone treatment can suppress the signals the brain uses to support sperm production.

Does Fertility Come Back After TRT?

Often, yes.

But not always quickly.

After testosterone is stopped, sperm production can take months to recover.

For some men, recovery takes much longer.

That is why you should not simply assume:

“I will stop TRT when I want a baby.”

If future fertility matters to you, talk about it before treatment begins.

Are There Other Options?

Sometimes.

Men who have low testosterone and want to preserve fertility may have other treatment options.

Depending on the cause, a specialist may consider medicines that help the body make more of its own testosterone instead of simply replacing it.

These treatments require medical supervision.

The Simple Rule

If you are trying to have children now—or may want children soon—tell your doctor before you start TRT.
