Cancelling or rescheduling
Please give at least 24 hours’ notice; call or text (515) 717-7648.
A late cancellation or a no-show may mean losing any booking fee or deposit for that appointment. We waive this for emergencies and illness.
If you arrive more than 15 minutes late, your appointment may need to be rescheduled.
Consultations and booking fees
Your first consultation is free and has no booking fee.
Appointments for paid treatments may require a $29 booking fee to reserve your time; we tell you before you book.
The booking fee is credited toward that treatment and refunded if you cancel with at least 24 hours’ notice.
Treatments, sessions and packages (all devices and services)
Completed treatments are not refundable, unless we made an error.
Prepaid sessions must be used within 6 months of purchase, unless a different period is stated when you buy them. During that period, unused sessions are refundable: each completed session is charged at its standard single-session price and the rest of what you paid is refunded. After the period ends, unused sessions are not refundable.
If a clinician advises you to stop treatment for a medical reason during that period, unused sessions are refunded at the price you paid for each one.
Introductory offers (for example, a first-session special) follow the same rules.

Medication
WellForm MD does not sell medication. Your pharmacy dispenses it and bills you for it, and its own refund policy applies.

Approved refunds go back to your original payment method within 10 business days.
Payments financed through CareCredit or Cherry are refunded through that lender.
