SMS Terms

How the WellForm MD text-messaging program works: what we text about, how often, costs, and how to get help or opt out.

The program

WellForm MD text messages are sent by WELLFORM MD PLLC ("WellForm MD"). Texts come from (515) 385-0071.

What we text you about

Appointment confirmations and reminders, account and billing notices, customer-care replies from our team to questions you send us, and notices that a secure message is waiting for you in the patient portal.

With your consent, we may also send limited promotional texts, such as occasional offers and program updates.

Texts never include health details; for those we point you to the secure patient portal.

How to opt in

You opt in by entering your mobile number and checking the optional text-message box on the WellForm MD consultation request form at wellformmd.com/booking/. The box is never pre-checked and you can request a consultation without checking it. You can also opt in by texting us first.

The checkbox reads: "Yes, text me. I agree to receive text messages from WellForm MD at the mobile number I provided, including appointment reminders, account and customer-care messages, and occasional promotional offers. Consent is optional and not a condition of purchase. Message frequency varies. Msg & data rates may apply. Reply HELP for help, STOP to opt out. See our SMS Terms and Privacy Policy."

Consent to receive texts is not a condition of any purchase.

Cost

Msg & data rates may apply. Check your mobile plan for details. WellForm MD does not charge for texts.

How to stop

Reply STOP to any text to opt out. END, CANCEL, UNSUBSCRIBE, QUIT and STOPALL also work. You will receive one text confirming you are unsubscribed and no further texts after that. To join again, reply START or UNSTOP.

Not for emergencies

Do not use text messages for an urgent medical need. If you have an emergency, call 911.

Carriers

Supported on major U.S. wireless carriers. Carriers are not liable for delayed or undelivered messages.

Privacy

We do not sell or share your mobile number, text-message opt-in data or consent with third parties or affiliates for marketing or promotional purposes. Our Privacy Policy explains how we handle your information.
