Why weight loss slows
If your weight trend really has flattened, the most important thing I can tell you is this: a slowing curve is not evidence that something is broken. It is what the curve of successful weight loss looks like.
You have probably heard the popular explanation — "you wrecked your metabolism." Patients arrive with that sentence already installed, and it does real damage, because it turns a predictable physiological process into a personal verdict. The truth is less dramatic and far more useful. Several ordinary forces bend the curve, and they bend it in everyone.

A smaller body runs on less
The first force is arithmetic. A smaller body simply costs less energy to run and to move. The energy plan that produced steady loss at your starting weight produces slower loss twenty pounds later, because the gap between what you take in and what you use has quietly narrowed. Nothing failed. The target moved — because you moved it, in the direction you wanted.
Your body adapts on purpose
The second force is adaptation. When weight falls, the body does not passively accept the new state; it economizes. Energy expenditure drops somewhat further than the new, smaller body size alone would predict — in studies of people maintaining a reduced weight, roughly ten to fifteen percent further. Researchers call this adaptive thermogenesis. I call it your body being good at its job. For most of human history, defending against weight loss was a survival feature, not a bug.
Appetite signals push back
The third force works on the other side of the ledger. The hormones that regulate hunger and fullness shift with weight loss — hunger signals get louder, satiety signals get quieter — and the effect can persist long after the scale stops moving. If you have noticed that the same discipline now takes more effort, you are not imagining it and you are not weaker than you were. The signal you are pushing against is genuinely stronger.
Life quietly renegotiates
The fourth force is the one nobody wants to own, so let me own it for my patients: behavior drifts. Portions creep. The food log gets abandoned on weekends. The new job eats the gym schedule. None of this is a moral failure — it is what happens when a demanding routine meets a long life — but it bends the curve just as surely as biology does, and unlike biology, it is fully reviewable. That review is exactly what Chapter 3 is for.
Where the curve flattens
Put those four forces together and you get the shape seen over and over in weight-loss research: steady early loss, then deceleration, then a long flat stretch. In structured lifestyle programs, the flattening typically arrives around six months. In the large trials of modern anti-obesity medications, average weight follows the same shape — a longer, deeper descent, then a leveling — which we will look at directly in Chapter 4.
A plateau, in other words, is not the opposite of success. It is the usual sequel to it. The body has found a new equilibrium at a lower weight and is defending it. The question that matters is not "what did I break?" but "is this equilibrium the right place to stop — and if not, what single thing deserves to change?"
