Terms of Service
Review the terms governing appointments, payments, communications, and use of WellForm MD services.

By using the services of WellForm MD (“we,” “our,” “us”), you acknowledge and agree to these Terms of Service. Please read them carefully before receiving any services. These Terms apply to all services, whether accessed in person, online, or through third-party scheduling or communication platforms.
Your use of our services is also governed by our Privacy Policy, which explains how we collect, use, and protect your personal information. By agreeing to these Terms of Service, you also acknowledge that you have read and understood our Privacy Policy.
1. Services Offered
WellForm MD provides elective aesthetic and wellness treatments, including but not limited to body sculpting, wellness coaching, red light therapy, and other non-medical wellness services. These treatments are not a substitute for professional medical care and should not be used as such.
2. Client Responsibilities
Clients agree to:
Provide accurate and complete medical and personal information prior to treatment.
Follow all pre- and post-treatment instructions as provided by our staff.
Inform WellForm MD of any health status changes before receiving services.
Acknowledge and accept potential risks associated with elective wellness and aesthetic treatments.
3. Appointment Policies
Cancellations & No-Shows
Clients must cancel or reschedule appointments at least 24 hours in advance.
No-shows or last-minute cancellations may incur a fee of $50.
Repeated cancellations or no-shows may require a non-refundable deposit for future appointments.
Late Arrivals
If a client arrives late, the appointment may be shortened or rescheduled at our discretion.
Arriving more than 15 minutes late may result in automatic cancellation.
4. Payment Policies
Except where otherwise noted, all services are not covered by insurance and must be paid out of pocket.
Payment is due at the time of service.
Accepted payment methods: Credit Cards, Debit Cards, and CareCredit.
No refunds will be issued for completed services unless an error occurs on our part.
Prepaid packages and memberships are non-refundable but may be transferred at our discretion.
Clients are responsible for any declined payments or chargeback fees.
5. Medical Disclaimer
WellForm MD provides medical services related to weight management but does not serve as a primary care provider. Clients should maintain a relationship with their primary care physician for general medical needs.
Our services do not diagnose, treat, or cure any medical condition outside of weight management.
Clients should consult a healthcare provider before undergoing any wellness treatments.
Results vary by individual, and no guarantees are made regarding treatment outcomes.
Clients must sign an Informed Consent Form before receiving certain treatments.
For all medical services outside of weight management that require insurance coverage, clients must consult with their primary care providers or specialists.
6. Limitation of Liability
WellForm MD is not responsible for dissatisfaction with results or any adverse reactions that may occur.
We are not liable for indirect, incidental, or consequential damages, except in cases of gross negligence or intentional misconduct.
Clients assume full responsibility for their decision to undergo treatments and agree to release WellForm MD from any liability related to elective treatments.
7. Intellectual Property
All content on our website, promotional materials, and branding—including text, images, and logos—is the property of WellForm MD.
Unauthorized use, reproduction, or distribution of WellForm MD’s content is strictly prohibited.
8. Governing Law & Dispute Resolution
These Terms are governed by the laws of the State of Iowa.
Any disputes shall first be resolved through mediation or arbitration before litigation.
If legal action is necessary, it must be filed in a court located in Iowa.
9. Changes to These Terms
WellForm MD reserves the right to update these Terms of Service at any time. Changes will be posted on our website with the updated effective date. Continued use of our services after updates constitutes acceptance of the new terms.
10. SMS Communications & A2P 10DLC Compliance
WellForm MD utilizes Application-to-Person (A2P) 10-Digit Long Code (10DLC) messaging to communicate with clients via SMS. This includes both transactional and, with explicit consent, promotional messages. We are committed to adhering to the guidelines established by the Cellular Telecommunications Industry Association (CTIA) and other relevant regulatory bodies.
Types of Messages
By providing your mobile number and opting in, you consent to receive SMS messages from WellForm MD, which may include:
Appointment confirmations and reminders
Rescheduling or cancellation notices
Important updates regarding your scheduled appointments
Promotional offers, service announcements, wellness tips, and special event notifications (with express consent)
You will only receive promotional messages if you have explicitly opted in through our web forms or other consent mechanisms.
Message Frequency and Rates
Message frequency varies based on your appointment activity and consent preferences. Standard message and data rates may apply according to your mobile carrier’s terms.
Opting Out
You may opt out of SMS communications at any time by:

Upon opting out, you will no longer receive appointment or marketing-related text messages. You may opt back in at any time.

Message and data rates may apply. You may opt out of SMS communications at any time by replying “STOP” to any message. You may unsubscribe from emails by clicking the “Unsubscribe” link provided in each message.
